Предмет закупівлі:	Капітальний ремонт будівлі амбулаторії з перепрофілюванням її в приміщення стаціонарного догляду за особами, які втратили здатність до самообслуговування чи не набули такої здатності за адресою: пр. Шевченка, 24, с. Великий Глибочок, Білецької громади, Тернопільського району, Тернопільської області (з меблями та технікою)
Валюта розрахунків:	Оплату послуг буде здійснено в національній валюті України у безготівковій формі.
Підписання договору:	За результатом проведеної процедури з Переможцем буде підписано два тристоронніх договори: – на виконання робіт із капітального ремонту; – на постачання, встановлення (монтаж, підключення) техніки та меблів. Важливі положення Договору: Ціна та обсяг – Вартість, перелік та обсяги робіт, техніки і меблів, зазначені в комерційній пропозиції виконавця, є фіксованими та не підлягають зміні за будь-яких обставин. Терміни – Усі передбачені Договором роботи повинні бути виконані в повному обсязі до дати його закінчення. Можливість продовження строку дії Договору не передбачається.

Термін подання тендерних пропозицій:	до 06.09.2025 року до 18:00 включно на ел.адресу [email protected] з темою листа «ТЕНДЕР_НАЗВА_ВИКОНАВЦЯ_Капітальний ремонт будівлі амбулаторії з перепрофілюванням її в приміщення стаціонарного догляду за особами, які втратили здатність до самообслуговування чи не набули такої здатності за адресою: пр. Шевченка, 24, с. Великий Глибочок, Білецької громади, Тернопільського району, Тернопільської області (з меблями та технікою)».
Вимоги до виконавця:	1) До участі у конкурсі запрошуються суб‘єкти підприємницької діяльності, належним чином зареєстровані згідно із законодавством України; мають досвід роботи у відповідній сфері не менше ніж 2 роки, мають відповідні КВЕДи, які відповідають запиту; 2) Виконавець має бути зареєстрований на підконтрольній території України; 3) Має можливість виконати роботи та здійснити доставку та установку товарів у вказаний в договорі строк та можливість надати оригінали супроводжуючих документів з печаткою та підписом (договір на виконання робіт, договірну ціну, кошторисну документацію з усіма додатками, рахунок-фактуру, акт виконаних робіт, тощо); 4) Надасть гарантію мінімум 3 роки на ремонтні роботи. 5) Має дозволи на виконання робіт підвищеної небезпеки. 6) Перед подачею тендерної пропозиції Виконавець повинен оглянути будівлю, що підлягає ремонту та оцінити обсяги робіт, які необхідно виконати у встановлений термін 26.11.2025 року. На підтвердження проведеного огляду в складі тендерної пропозиції необхідно надати Акт огляду за підписом представника балансоутримувача приміщення. Дана умова є обов’язковою, так як у ході виконання умов договору не допускаються будь-які зміни обсягу робіт та вартості товарів. 7) Запропонована техніка та меблі повинні бути новими, якісними та таким, що раніше не використовувався. Товар повинен бути виготовленим не раніше 2023 року, непошкодженим, не мати дефектів, повністю бути придатним до використання, а також таким, що не перебував в експлуатації або не був відновленим. Товар повинен бути укомплектованим відповідно до комплектації виробника та заявленої учасником. Пакування має бути не пошкоджено. 8) Ціна пропозиції (товару) включає в себе всі супутні витрати Постачальника, пов’язані з цією закупівлею: тара та упаковка, транспортування (доставка), завантаження та розвантаження товару, доставка безпосередньо до місця встановлення товару, збирання, встановлення, підключення та всі інші витрати, що мають бути здійснені Виконавцем. 9) Якщо товар виявиться неякісним або таким, що не відповідає технічним (якісним) умовам, Виконавець зобов’язаний його замінити протягом 10-ти робочих днів. Всі витрати, пов’язані із заміною товару неналежної якості несе Виконавець. 10) Гарантійний строк техніки та меблів: згідно гарантійного строку заводу-виробника, але не менше ніж заявлено у технічних вимогах. 11) Наявність Сервісних центрів для обслуговування техніки в м. Тернопіль.
Main (mandatory) documents of the Proposal:	1. The commercial offer must contain: * Contract price (firm) in accordance with the “Guidelines for determining the cost of construction” dated 01.11.2021 No. 281, drawn up according to the sample. The contract price is determined unchanged and is not subject to adjustment until the contract is fully executed. * Local estimate * Consolidated statement of resources, * Calculation of general production costs * Explanatory note to the estimate * Defective act and file in the AVK software complex or another that interacts in terms of data transfer. 2. The consolidated statement of resources must have the full name of the materials (goods), trademark, brand, quantity of goods in the package, unit of measurement; 3. The cost of the work includes: * the entire complex of works in accordance with the defective act (design documentation), technology of work execution, technological maps; * operation of machines, mechanisms, tools that will be used in the performance of work, costs for their relocation, maintenance, cost of fuels and lubricants; * installation, dismantling of scaffolding, formwork, devices, costs for their relocation, maintenance; * transportation and arrangement of the building site, costs for its operation; * unloading and loading work, movement of building materials, structures and equipment around the construction site; * costs for business trips, accommodation of employees; * general production costs; * administrative costs and estimated profit; * cost of materials associated with the performance of work; * cleaning of the construction site after completion of work (cleaning of garbage, waste, unnecessary material resources, structures, etc.); * cost of taxes; * cost of furniture with delivery, assembly and placement at the site; * cost of equipment with delivery, placement at the site and connection to networks. 4. The tender offer for the purchase of equipment and furniture must contain the following information: * Full name of the product, including the manufacturer, trademark, brand; * Product characteristics (quantity of goods in the package, weight, volume in ml. if necessary, etc.); * Quality certificates for products, if required by the Legislation of Ukraine. The goods must comply with international quality and safety standards; * Price per unit of goods and the total amount, according to the Appendix(es) to the Request for Proposals; * Payment terms; * Terms and delivery terms; * Offer validity periods. 5. The tender offer must be drawn up in Ukrainian and sealed with a signature and seal – contain data on the full cost of the Contract, terms of work, terms of delivery of furniture, equipment and terms of payment (according to the sample).
	6. Реєстраційні документи: Для ФОП: - Виписка з ЄДРПОУ. - Копія витягу з реєстру платників єдиного податку (якщо учасник є платником єдиного податку). - Копія довідки/свідоцтва платника ПДВ і витяг з реєстру платників ПДВ (для платників ПДВ). Для юридичних осіб: - Виписка з ЄДРПОУ. - Статут (остання редакція); - Документ, що підтверджує повноваження підписанта (наказ про призначення, довіреність, витяг з протоколу загальних зборів тощо). - Копія довідки/свідоцтва платника ПДВ і витяг з реєстру платників ПДВ (для платників ПДВ). - Копія витягу з реєстру платників єдиного податку.
	7. Additional documents: ● Certificate of absence of grounds for refusal to participate in the procurement procedure, specified in parts 1 and 2 of Article 17 of the Law of Ukraine No. 922, in particular, the commission of criminal offenses committed for selfish motives, the commission of anti-competitive actions, corruption offenses, the commission of offenses related to the use of child labor or any forms of human trafficking, the absence of arrears in the payment of taxes and fees (mandatory payments); ● Documents on financial reporting to assess the financial capacity of the Participant: – Tax return for the last 12 months. (if the reporting period of the Participant for the last year has not begun, then the Participant shall submit reporting for 6 months of the current year); – Balance sheet and Report on financial results for the last 12 months; ● Documents confirming experience: – Reference letter with a list of implemented similar agreements (contracts). The participant provides a table with a list of at least 3 executed agreements over the past 24 months, containing: * client name; * brief description of the agreement; * amount of the agreement; * number and date of the agreement; * payment terms. – Letters of recommendation regarding the implementation of similar agreements (contracts) (minimum 3 letters) over the past two years with a similar type of work; ● Calendar schedule of work and supply of equipment and furniture; ● Certificate of availability of employees with appropriate qualifications who have the necessary knowledge and experience. Attach to the Certificate document(s) for each employee (the document must indicate the employee’s last name and first name or the employee’s last name and initials, or the employee’s last name, first name, patronymic), indicated in the certificate, which certifies the possibility of using the labor of such an employee by the participant (for example: staff list / employment contract / service contract / copy of the work book (the first page containing information about the employee’s last name and first name, and the page containing the record of employment) / civil law contract / other document). ● Certificate of availability of material and technical base and construction machinery and equipment to perform the entire scope of work with supporting documents on ownership or the right of use (valid until 12/31/2025) ● Letter of guarantee on the compliance of your proposal with the technical task – defective act (project). ● A copy of the valid permit for performing high-risk work. ● A copy of the license or document of a permissive nature (if any) for conducting a certain type of economic activity, if obtaining a permit or license for conducting such activity is provided for by law. If obtaining a license is not provided for – a letter of explanation with a reference to the legislative act regulating this issue. 8. The validity period of the offer submitted by the Participant is at least 30 calendar days from the moment of opening the offers.
Якщо до комерційної пропозиції не буде прикріплено вищезгадані документи, то така пропозиція може бути не розглянута.
Additional documents to the Proposal: (not mandatory for participation but desirable for signing a cooperation agreement)	- A copy of the passport of a citizen of Ukraine (or an ID card with an extract from the demographic register) for an individual entrepreneur or an authorized person of an LLC/legal entity. – A copy of the identification number (RNOKPP), if available. – A certificate from a banking institution on opening an account or bank details. – A letter of guarantee on the fulfillment of obligations in accordance with the terms of the tender procedure. – A statement of the movement of funds on a bank account (for a period of at least the last six months) to confirm financial capacity. – A letter of guarantee that materials for repair work, furniture and equipment are not produced in the territory of the Russian Federation and/or the Republic of Belarus and are not imported/exported to/from the territory of the Russian Federation and the Republic of Belarus.
У разі наявності рівнозначних пропозицій (однакова ціна, строки виконання та умови оплати), перевага надаватиметься учаснику, який надасть повний пакет додаткових документів, передбачених вимогами цього запиту.
The main criteria for the qualification selection of Contractors:	Availability of the necessary KVED to provide services according to this Request. Provision of a full package of basic documents specified in this Request; Registration in the territories controlled by Ukraine;
The main criteria for evaluating a commercial offer:	Compliance of the proposed works and goods with the characteristics specified in the Request; Provision of a full commercial offer (for all items) according to the Request; Price; Giving preference to offers with deferred payment
Additional conditions:	By submitting a commercial offer, the Contractor agrees to the processing, use, dissemination and access to personal data, as provided for by the Law of Ukraine “On Public Procurement”, as well as in accordance with the norms of current legislation, personal data (including passport data, identification code, electronic identification data: telephone numbers, email addresses or other necessary information provided for by law), information that the Contractor provides about himself to ensure participation in the procurement procedure, civil and economic relations. By submitting its offer, the Contractor declares and guarantees that it and all its subcontractors will protect people from sexual harassment and sexual exploitation, the term “sexual exploitation” means any abuse or attempted abuse of a vulnerable position, power or trust for the purpose of sexual exploitation, including, but not limited to, obtaining monetary, social or political benefit from the sexual exploitation of others. Similarly, the term “sexual harassment” means a physical act or threat of a sexual nature, whether with the use of force or under unequal or coercive conditions.”
БО БФ “Посмішка ЮА” залишає за собою право:	• змінювати обсяг закупівлі в межах +/- 20% від початкового обсягу без зміни вартості за одиницю послуги. • запитати додаткові або підтверджувальні документи. • повністю або частково відхилити будь-яку комерційну пропозицію у разі виявлення суттєвих недоліків у її змісті, невідповідності вимогам Запиту, а також у разі наявності репутаційних ризиків, що можуть виникнути в процесі або в результаті подальшої співпраці з учасником. • вступати у переговори з обраним постачальником або доручити виконання контракту більш ніж одному Виконавцю.

Контракт із переможцем відповідної закупівельної процедури може бути розірваний у випадку виявлення фактів корупційних порушень під час підписання угоди за результатами закупівлі або в процесі виконання умов договору. БО БФ «Посмішка ЮА» докладає зусиль із запобігання, виявлення та вжиття заходів проти всіх випадків шахрайства та зловживань.
